NO ON-SITE REGISTRATION
NO REFUNDS will be processed until 30 days after the district meeting, which must be indicated via email with original receipt.
Vending booths will be assigned by the Vendor coordinator in the order in which they are sold.
Vending booth space is a standard 10ft by 10ft space with pipe and drape (3ft on the sides and 8ft in the back).
Each booth will include One (1) 8ft table and two (2) chairs.
A maximum of 50 vendor booths will be sold.
No power will be supplied.

PLEASE NOTE: If selling Omega Psi Phi Fraternity, Inc. paraphernalia you must provide the current license vendor identification number issued by Omega Psi Phi Fraternity, Inc. on this form. Failure to comply with each requirement will result in a denial of the privilege to sell at scheduled meetings and functions hosted by Seventh District of the Omega Psi Phi Fraternity, Inc. Any vendor selling Omega paraphernalia that depicts the canine image or likeness will be closed and prohibited from further sales at the function. Additionally, the vendor will not be refunded any of the fees paid to the Seventh District of Omega Psi Phi Fraternity, Inc.

By completing the on-line registration Card Holder acknowledge receipt of goods and or services in the amount of the total shown and agrees to perform the obligation set forth in the Card Holder's agreement with the issuer of said card. Request for refunds must be sent to the 7^th District KRS in writing or via email by March 23, 2025.
